European Specialty Examination in Gastroenterology & Hepatology

From January 2018 the Specialty Certificate Examination (SCE) in Gastroenterology and the European Section and Board of Gastroenterology and Hepatology Examination (ESBGHE), have been replaced by the European Specialty Examination in Gastroenterology and Hepatology (ESEGH).
The ESEGH has become the General Medical Council (GMC, UK) approved mandatory summative assessment of knowledge for UK trainees in Gastroenterology and Hepatology. From 2018, trainees who hold MRCP(UK), the ESEGH, and gain a certificate of completion of training (CCT) in the UK will be entitled to apply for the postnominal MRCP(UK) (Gastroenterology).
For European based candidates, the examination is a demonstration of having achieved the knowledge necessary to fulfil the requirements of the ESBGH curriculum which is a Europe-wide description of the level expected of a specialist in gastroenterology and hepatology. The ESEGH is mandated for trainees in the Netherlands and Switzerland.
Candidates from Europe, UK, and the rest of the world can take this high quality examination which ensures that certified specialists have sufficient knowledge of gastroenterology and hepatology.
The ESEGH is delivered once a year. See the table for key dates for the next examination below:

Eligibility requirements

There are no entry requirements for the ESEGH although candidates in UK training posts would normally take the examination as part of their higher specialty training. The European Specialty Examination in Gastroenterology and Hepatology Board have reviewed the performance of candidates in the United Kingdom and now advise UK based trainees dual accrediting in General Internal Medicine and Gastroenterology to attempt the examination during ST5 in order to ensure that they have time for 3 attempts prior to CCT. Those in single accreditation programme may wish to consider a first sitting in their ST4 year.
EU candidates would normally take the examination towards the end of their specialization.
Please note, if you have passed the SCE in Gastroenterology or the European Section and Board of Gastroenterology and Hepatology Examination, you are not permitted to apply for the ESEGH.

Google LLC^[5] is an American multinational technology company that specializes in Internet-related services and products, which include online advertising technologies, search engine, cloud computing, software, and hardware. Google was founded in 1998 by Larry Page and Sergey Brin while they were Ph.D. students at Stanford University in California. What is Part 1?

Purpose and objectives

Part 1 is the entry-level examination accessible to doctors with a minimum of 12 months' postgraduate experience in medical employment. It covers a broad range of topics to ensure the level of knowledge is appropriate to physicians at the beginning of postgraduate training.
MRCP(UK) tests the acquisition of a representative sample of medical knowledge, skills and behaviour as specified in the UK Specialty Training Curriculum for Core Medical Training. Part 1 is the first component of a sequence of assessments intended to match the progression of trainees undertaking the Core Medical Training programme in the UK, adding unique information and building on previous assessments.

Knowledge as an essential base for the practice of clinical reasoning

Knowledge is vital as a basis for learning during postgraduate training, to develop understanding of disease processes and their treatment. Building on knowledge of basic medical science gained in undergraduate education, the purpose of Part 1 is to test knowledge and understanding of common and important disorders, as outlined in the UK curriculum, as well as clinical science. Knowledge and understanding of UK national guidelines is also required.
Success in the Part 1 Examination indicates that the trainee has not only retained the knowledge acquired during undergraduate training, but that his/her knowledge of medicine has expanded and kept pace with developments that have occurred since graduation, and continues to provide an appropriate basis for clinical decision making. Part 1 is a valuable milestone during training, and helps to prepare trainees for the more challenging Part 2 Written and Clinical Examinations.
Clinical science forms the basis of all medical practice, and so it is an important component of the Part 1 exam. Wherever appropriate, clinical science questions will include a clinical stem, although this will not invariably be the case if such a stem would not be justified due to the nature of the question.

Continuing Professional Development for examiners

Physicians are responsible for keeping themselves up-to-date in all areas of their practice. Continuing Professional Development (CPD) enables physicians to demonstrate that they are maintaining their skills in their practice.
The Federation of the Royal Colleges of Physicians of the UK awards CPD credits for MRCP(UK) and Specialty Certificate Examination (SCE) examining activities. The Federation has agreed to award up to 12 external credits per year for examining activities.

CPD certificates

CPD certificates are distributed once a year, in late May/early June; however, examiners are encouraged to record their examining activity in their CPD diary throughout the year.

How to claim CPD credits

To add MRCP(UK) activities as external credits to your CPD diary follow these steps:

1. Go to the 'Add Self Certified Entry' tab of your CPD diary
2. Click on the ‘MRCP(UK)’ tab
3. Enter the date the activity took place, activity details and the number of credits, and select the MRCP(UK) activity
4. Click on ‘Add this activity to your diary’
5. Complete a reflection
6. Click on ‘Finish’.

The first 12 credits from MRCP(UK) activities will be counted as external credits and the remaining credits counted as personal credits. All Trainee Examiner activities must be counted as personal credits.

International development

The MRCP(UK) Diploma and SCEs are now offered in more than 100 locations around the world with over 20,000 doctors taking them each year. As part of this effort, we work in partnership with many hospitals, universities, colleges, the British Council and governmental bodies around the world in delivering the examinations.
MRCP(UK) examinations are mandatory for progression through core or specialty training in the UK. Indeed, in some countries outside the UK they also form a mandatory part of training and assessment (Hong Kong, Iceland and Singapore). In other countries, MRCP(UK) examinations represent an alternate means of  demonstrating knowledge and skills, in conjunction with local training programmes (Malaysia). Through our interactions with international partners we have developed extensive understanding of the training requirements for doctors in many countries and work closely with local employers and regulators to refine the support we provide.
MRCP(UK) and our partner organisation, the Joint Royal Colleges of Physicians Training Board (JRCPTB), have started working together to build comprehensive packages of training, assessment and curricula, which can be delivered anywhere in the world. We are looking to form strong partnerships with international colleagues and authorities in the development of postgraduate medical education in their home countries. This is part of our belief that while our examinations are highly valuable as stand-alone benchmarks of knowledge and skills, they can also form part of a fully accredited training programme outside the UK.

PACES Awards

The Federation of Royal Colleges of Physicians of the UK relies on many organisations and individuals to give up their time and resources in the delivery of PACES every year. The following awards are given in recognition of this commitment.

PACES Plaques

The inaugural plaques were awarded in 2011 to mark the ten year anniversary of PACES. The aim was to recognise those organisations who have been most involved in the examination, hosting a high number of candidates or cycles, and provide an aspirational target to new centres.
Plaques are awarded every three years and are sent with a letter of recognition to the Medical Director, CEO and Host Examiner at each centre, from the MRCP(UK) Medical Director, Clinical Examining Board Chair, and the International Associate Medical Director for international centres.
In order to be eligible, our UK centres must have, in the previous three years, assessed more than 90 candidates or hosted the examination for seven days or more.  International centres receive a plaque if they have hosted an examination in the last three years, in recognition of capacity challenges in our overseas centres.

PACES Champions

MRCP(UK) on behalf of the Federation has developed the new PACES Champion awards to honour individuals for their commitment to PACES. One UK and one international recipient will be chosen each year from your nominations.
Nominations can be made by anyone who is involved in PACES including MRCP(UK) staff – although self-nominations will not be accepted.  The form for nomination is available at the link below. Examples of individuals who can be nominated include centre support staff, medical professionals or volunteers who do not receive direct payment for their involvement.
Nomination can be made under one or more of the following criteria:

a. Overall contribution to PACES
b. Longstanding dedication to PACES
c. Contribution as a team player
d. Positive attitude in supporting PACES
e. Supporting PACES in challenging circumstances 

The nomination form includes a section to provide examples of the nominee’s contribution. This statement, and the supporting statement provided by a Senior Examiner, Senior Administrator or the International Associate Medical Director (IAMD), will provide the basis for the judging panel’s decision.

Question writers

Become a Question Writer

Writing ‘Best of 5’ questions in your specialty is a great way to become involved in MRCP(UK) examinations. We have an annual question writing meeting for both MRCP(UK) Part 1 and Part 2 written examinations, and  a separate annual meeting for the 11 Specialty Certificate Examinations (SCEs). Each meeting is held over 2 days, in an excellent conference centre at Warwick University in Coventry. Our current question writers value working with colleagues and the quality of the CPD gained during the meetings. We also have a relaxed dinner at the end of the first day. Question writers include trainees who have passed the relevant examination and both junior and senior consultants in the specialty.
For MRCP(UK) Parts 1 and 2, each Specialty Question Group (SQG) has up to 12 members and each SCE Question writing group (QWG) has up to 25 members. SQG and QWG members draft new questions in their specialty, which are then discussed at the two-day peer review meetings. SQGs are held in November, and QWGs in March/April.

Benefits

The meetings offer excellent networking opportunities, providing interaction with physicians from across the UK. Members can claim external CPD credits for attendance. Current and previous question writers are often recruited to examination boards and standard setting groups because of the additional expertise they bring.

Eligibility

If you meet the following criteria you are eligible to apply:

• Member or Fellow of one of the Royal Colleges of Physicians of the United Kingdom (or another UK medical Royal College)
• GMC registered with a License to Practice and in good standing
• Actively engaged in the training of junior doctors (or within the last two years)
• Up-to-date with employer’s equality and diversity training, and appraisal process/ CPD and with relevant National Guidelines
• Consultant or specialty trainee (who has passed the relevant SCE for QWG vacancies) practising in some aspect of clinical medicine, either with a sub-specialty or area of interest relevant to a specific SQG, or in Acute/Emergency Medicine or any of the 11 SCE specialties

Training and support

If your application is successful you will be invited to attend a one-day training session at the RCP London, after which you can attend your first SQG or QWG meeting. All personal expenses to cover the costs of attending SQG or QWG meetings will be met by MRCP(UK) in line with the Federation expenses policy within the UK.

Responsibilities of an SQG or QWG member

As a question writer you will:

• draft questions for the MRCP(UK) Written Examinations or the Specialty Certificate Examinations as directed by the appropriate Examining Board
• ensure that the academic objectives in producing assessment materials for the examination are met
• assist in any review of the question writing process to ensure that sufficient questions of quality are produced for the examination
• ensure that the assessment material remains up to date and relevant to the curriculum and meets the standards required by the General Medical Council (GMC).

Appointments are normally for five years, subject to satisfactory performance during a probationary year. Question writers must draft the required number of questions (including selected topics that are identified in a commissioning letter in advance) and be available to attend the once-yearly meetings. Board members

The academic quality of the MRCP(UK) Diploma and Specialty Certificate Examinations is an accomplishment to be proud of – and to protect. Guarding these standards is the remit of our Examining Boards, which include the:

• MRCP(UK) Part 1 Examining Board
• MRCP(UK) Part 2 Written Examining Board
• MRCP(UK) Part 2 Clinical Examining Board (for PACES)
• Scenario Editorial Committee
• Specialty Certificate Examination Boards (one for each of 12 specialties).

Ensuring each examination is updated regularly, in line with the current UK curriculum and requirements of the General Medical Council, is a key responsibility for the Boards. They also work closely with question and scenario writers, to see that the questions are pitched at the right level of difficulty. The Boards determine the content of each examination and approve the pass mark.
The standard setting groups advise on the difficulty and content of the examinations, and agree the pass mark. If you work closely with trainees and know what level of knowledge is required for a new registrar or consultant, this might be the job for you.

Exam roles vary to suit interest and time commitments but all come with the dedicated support of the staff in the examinations departments and central office.
The eligibility criteria for these roles are:

• Member or Fellow of one of the Royal Colleges of Physicians of the United Kingdom
• GMC registered with a licence to practise and in good standing
• Currently involved in training with at least 2 years' experience
• Up-to-date with employer appraisal process
• Up-to-date with equality and diversity training
• Physicians interested in, but not necessarily expert in, medical assessment.

Training and support

All volunteers are trained through a formal induction programme and receive the support of the staff in the examinations departments and central office throughout their involvement in the exams. Advice on where to access equality and diversity training can be obtained from the examinations office of your parent college.

First sub-Saharan PACES exam takes place in Nairobi, Kenya

The Federation of the Royal Colleges of Physicians of the UK is delighted to announce the successful launch of the MRCP(UK) PACES examination in Aga Khan University’s Medical College (AKU), Nairobi, Kenya. This is the first location in Sub-Saharan Africa to offer the exam and the 25th MRCP(UK) international PACES centre. This follows the introduction of the MRCP(UK) Part 1 written exam in October 2018.

We are pleased to announce that the General Medical Council (GMC) has approved our proposed changes to the MRCP(UK) PACES examination.

It has been an interesting and enjoyable five years with candidate numbers continuing to grow substantially around the world. I have been particularly pleased that we have reduced the cost of some of our examinations, developed new examinations for European specialists, and taken MRCP(UK) into new partner countries, including the USA. MRCP(UK) PACES Musgrove park cancelled due to severe weather

We wish to advise that the MRCP(UK) PACES examination at Musgrove Park Academy, Musgrove Park Hospital, due to take place on Saturday 3 and Sunday 4 March 2018 has been cancelled. South West England has been escalated to a red weather warning which is a ‘risk to life’ snow warning. This decision has not been made lightly but we have a duty of care to our candidates, patients and examiners and for their own safety we cannot ask them to travel in such adverse weather.
